ACTION: Notice.

    The Department of Defense has submitted to OMB for clearance, the 
following proposal for collection of information under the provisions 
of the Paperwork Reduction Act (44 U.S.C. Chapter 35).
    Title, Associated Form, and OMB Number: Health Professions 
Accession Forms; AETC Forms 1402, 1437; OMB Number 0701-0078.
    Type of Request: Reinstatement.
    Number of Respondents: 3,600.
    Responses Per Respondent: 1.
    Annual Responses: 3,600.
    Average Burden Per Response: 1 hour.
    Annual Burden Hours: 3,600.
    Needs and Uses: The information collection requirement is necessary 
for use by field recruiters in the processing of health profession 
applicants applying for a commission in the United States Air Force. 
Respondents are civilian candidates applying for a commission as 
healthcare officers. These forms provide pertinent information to 
facilitate selection of candidates for a commission.
    Affected Public: Individuals or households.
    Frequency: On occasion.
    Respondent's Obligation: Required to obtain or retain benefits.
    OMB Desk Officer: Mr. Edward C. Springer.
